The Idaho Health Guide would like to assist you in finding health care resources that our available for your family in Eastern Idaho.  The Idaho Health Guide has formed a partnership with the American Public Health Association and our objective is to "Build a Foundation for a Healthy Idaho".  Please enter your information (Name, Address, Phone Number and E-mail) and check any boxes that you or your family would be interested in obtaining more information about in your community.  Your information will be made available for vendors participating in the Eastern Idaho Health Festival to assist you or your family in finding or providing health care services.
